- Bachelor’s degree certificate.
- Graduation certificate from a Master’s or Doctoral program in Restorative Dentistry.
- Complete academic transcript for Master’s and Doctoral studies.
- Completed clinical logbook certified by the Program Director.
- Approved curriculum applied during the study period.
- Valid professional classification not less than Registrar (Resident).
- Clinical training duration must not be less than three on-site training years.
