3. Submit the completed form and documents via the designated SCFHS email for private health training center licensing.
4. SCFHS reviews the application and verifies that all requirements are met.
5. If any documents are missing, the applicant is notified to provide the required information.
6. Once the application is complete, a site visit is scheduled to verify the readiness of the training center.
7. The evaluation committee conducts the assessment and submits their report to SCFHS.
8. SCFHS issues a decision of approval, rejection, or a request for further requirements.
9. Upon final approval, the applicant pays the licensing fees.
10. The final license is issued and sent to the applicant via email.
