Licenses for Private Health Training Centers

1. Download and complete the licensing application form from the SCFHS website.

2. Prepare all required documents according to the type of license (initial, final, renewal, ownership transfer).

3. Submit the completed form and documents via the designated SCFHS email for private health training center licensing.

4. SCFHS reviews the application and verifies that all requirements are met.

5. If any documents are missing, the applicant is notified to provide the required information.

6. Once the application is complete, a site visit is scheduled to verify the readiness of the training center.

7. The evaluation committee conducts the assessment and submits their report to SCFHS.

8. SCFHS issues a decision of approval, rejection, or a request for further requirements.

9. Upon final approval, the applicant pays the licensing fees.

10. The final license is issued and sent to the applicant via email.

1. Completed application form for licensing a private health training center.

2. Commercial registration showing the training or health-related activity.

3. Copy of the national ID or residency permit of the owner/authorized person.

4. Lease contract or property deed of the training center facility.

5. Floor plan showing training rooms and facility layout.

6. List of trainers with qualifications and CVs.

7. Training plan outlining the proposed activities and programs.

8. List of available training equipment and resources.

9. Civil Defense safety certificate for the facility.

10. Safety and emergency plan for the training center.

11. Evidence of appropriate technical systems (attendance tracking, LMS, etc.).

12. Any additional documents required based on the type of request (initial/final/renewal).
